Birth/Death Certificates
Certified copies of certificates can be obtained

Effective July 1, 2003, anyone requesting certified copies of birth or death records will be required to show a Valid state issued identification card to prove their identity. When requesting records by mail or fax, a legible copy of a state Issued identification card must be included with the request. Credit card requests will no longer be accepted by phone. They Must be faxed to (775) 684-4156 or processed through the VitalCheck network at www.vitalchek.com for receipt via the internet.

Lost or Destroyed Marriage Certificate
If a marriage certificate has been lost or destroyed by accident by
the person solemnizing the marriage, he can go to the county clerk's
office and request a replacement certificate. The County Clerk in Churchill County charges $6.00 for the preparation of an affidavit of loss or destruction and issuance of the replacement marriage certificate. They will give the customer the form to take down to the Recorder’s office. The Recorder’s office will charge $19.00 ($14 recording fee and $5 certification fee).

Effective July 1, 2001 all financing statements

Financing Statements pertaining to Real Property must include the Assessor
Parcel number and the owner of record prior to recording. UCC's will be
accepted on a form prescribed and made available by the Nevada Secretary

- Metes and bounds descriptions need information on where legal
description was obtained. Previous recorded document information or
Surveyor name and address.
- Grantee address for tax purposes. Completed Declaration of Value
form.
