Health Nurse

Baby Your Community Health Nursing Services Office provides generalized public health nursing activities in counties other than Clark and Washoe. Services include community-focused health assessment, individual and group education for health promotion and risk reduction, and delivery of individualized preventative health clinical services in clinics, schools, homes and other community settings. 

The Churchill County Community Health Nursing Services Office is located at 85 North Taylor Street in Fallon (across the street from the County Offices Complex).

Most services require an appointment. To set-up an appointment, or check the date of the next immunization clinic, please call 775/423-4434.

HIV/AIDS Counseling/Education and Testing Services

Purpose
To provide HIV/AIDS counseling, education, and antibody blood testing.

Services

  • HIV/AIDS pretest counseling.

  • HIV/AIDS antibody blood testing.

  • HIV/AIDS post test counseling.

  • Public education concerning HIV/AIDS antibody blood testing.

  • Referrals to sources of care.

Eligibility
Available to anyone.

Fee
Minimum fee for blood test.

Cancer Screening Program

Purpose
To offer education about the importance of detecting the early signs of cancer and provide screening examinations.

Services

  • Health education

  • Breast examination

  • Pap Smear

  • Colorectal exam

  • Testicular exam

Eligibility
Available to women and men as appropriate.

Fee
Minimum fee is requested.

Children's Special Health Care Needs
& Special Children's Clinics

Purpose
To identify and provide services for children who may be physically handicapped or developmentally delayed.

Services

  • Referral to private physicians, if eligible.

  • Referral to specialized clinics such as Special Children's Clinics in Reno and Las Vegas.

Available at

  • Children's Special Health Care Needs - Newborn to 21 years of age

  • Special Children's Clinics - Newborn to 3 years of age, and in some instances older.

Fee
No charge for application. Clinic fees based on federal poverty level and sliding fee scale.

Dental Health

Purpose
To identify, educate and provide services for children who need dental care.

Services
If eligible, refer to dentists contracted with the dental health program.

Eligibility
Available to children 0 through 12 years of age.

Fee
No charge for individuals under 130% of federal poverty level. Sliding fee scale for all others.

Epidemiology

Purpose
To monitor and investigate communicable disease and provide information on prevention, treatment and control.

Services

  • Information on treatment, control and prevention of contagious disease.

  • Patient counseling.

  • Referral for medical treatment.

  • Health consultation to daycare centers and schools.

  • Investigation of vaccine reactions.

  • Local disease statistics.

  • Community education about communicable diseases.

Eligibility
Available to any patient, close contact or health professional.

Fee
A fee is requested for some services.

Family Planning

Purpose
To enable people to space the birth of their children in order to protect the health of the mother and the child.

Services

  • Information and education upon request.

  • Pregnancy testing and counseling.

  • Physical exam includes pap smear, pelvic exam, breast exam and limited lab tests.

  • Contraceptive services for women and men include forms of birth control, except for surgical procedures.

  • Supervision of methods of birth control.

  • Natural family planning, counseling and referral.

  • Sterilization counseling.

Eligibility
Available to anyone residing in Nevada.

Fee
Sliding scale for examinations and minimal fees for supplies are requested at time of service.

Healthy Kids (EPSDT) Screening

Purpose
Health screening for Medicaid recipients under 21 years old.

Services

Physical assessment and referral.

Health counseling.

Eligibility
Available from birth through age 20. Must be eligible for Medicaid through Nevada State Welfare.

Fee
No charge except for non-Medicare clients.

Immunization Clinic

Purpose
To protect children and adults from preventable, communicable diseases.

Services

  • Routine immunizations for infants and children starting at 2 months of age - Diphtheria, Pertussis, Tetanus, Polio, Measles, Mumps, Rubella, Hepatitis B and Haemophilus Influenza type b.

  • Adult vaccinations for Tetanus and Diphtheria.

  • Gamma Globulin for Hepatitis A contacts by doctor's orders.

  • Yearly flu vaccine for the elderly or those with chronic health problems.

  • Pneumonia vaccine for the elderly and those with chronic health problems.

Fee
A minimal fee for immunizations is requested but not mandatory.

Community Health Nursing Adult Education

Purpose
To provide skilled nursing services for the promotion of health in individuals, families and the community.

Services
Working with physicians and other health team members, Community Health Nurses provide:

  • Basic health teaching and guidance.

  • Mental health guidance.

  • Nutrition teaching and program information.

  • Communicable disease investigation.

  • Community referrals for health related services.

  • Health supervision and teaching in the home.

  • Measurement of, and education about, blood pressure.

Eligibility
Available to anyone

Fee
No charge.

Sexually Transmitted Disease Clinic

Purpose
To reduce the number of sexually transmitted diseases by early detection and treatment.

Services

  • Diagnosis and confidential treatment to any person, regardless of age.

  • Public education about sexually transmitted diseases - the signs and symptoms, and the importance of early treatment.

Eligibility
Available to anyone.

Fee
Minimal fees for exams, tests and for each prescription are requested.

Tuberculosis Clinic

Purpose
To eliminate tuberculosis through diagnosis and treatment of infected persons.

Services

  • Skin tests for the general public.

  • Treatment for persons infected with tuberculosis who have not yet developed the disease.

  • Screening for suspected cases: providing skin tests and X-rays, if necessary, to detect TB among family members and other contacts.

  • Treatment for patients with active TB.

  • Education for TB patients or contacts. 

  • Referral to other services.

Eligibility
Available to anyone.

Fee
No charge for "most" services.

Well Child Clinic

Purpose
To monitor growth and development, to note any signs of illness that might require a referral, and to encourage parents to enjoy and anticipate their child's development

Services

  • Counseling on good parenting health practices

  • Screening by measuring and recording mental and physical developments.

  • Physician referrals made if abnormal observations are found.

  • Childhood immunizations given when needed.

Eligibility
Available to 2-month old infants through 5-year old children.

Fee
Minimum fees for day care and Head Start exams and yearly fee for periodic well-child exams are requested.
