Mail Ballot Preference Form

With the passage of Assembly Bill 321 during the 2021 Legislative Session, all registered voters will receive a ballot by mail.  You can request to opt-out of receiving a mail ballot by submitting a mail ballot preference form.  You may also use the mail ballot preference form to request that your ballot be sent to an address other than you permanent residence or mailing address.

Completed forms can be mailed, emailed or faxed to the Churchill County Clerk/Treasurer's Office or you can update your mail ballot preference online at the NV Secretary of State's Voter Registration Services page

Application Deadline

If you wish to opt out of receiving a mail ballot the mail ballot preference form must be received by the clerk by the appropriate deadline.  Requests must be received by at least 60 days before an election to remove a name from the mail ballot list.  

First Time Voters

If you are a first time voter in Churchill County, and did not provide your identification at the time you registered, you must provide it with your mail.

Military and Overseas Residents

Military and overseas voters should use the Federal Post Card Application (FPCA) to register and/or request a mail ballot for voting. Uniformed and Overseas Citizens are also able to use the EASE system linked here. 

  • an active duty member of the Uniformed Services or Merchant Marine absent from your voting residence.
  • an eligible spouse or family member of an active duty member of the Uniformed Services or Merchant Marine.
  • a U.S. citizen residing outside the country.
